Sažetak
Generalized anxiety disorder (GAD) occurs two to three times more often in women than men. Somatic symptoms of anxiety (e.g. heart palpitations, chest pain, difficulty in breathing, sweating, chills, headache, nausea, etc.) are typical to other physical illnesses which increases the time to diagnosis of anxiety disorders and introduction of adequate therapy. Women are also more likely to develop the comorbid conditions, primarily depression. The GAD in comorbidity with depression most often has a chronic course and leads to impairment of overall functioning. Coexistence of GAD and depression indicates increased sensitivity which can be sex-specific factor along with sociocultural, environmental influences, and genetic predisposition. Biological background of GAD is in the dysfunction of central serotonin and GABA pathways, and dysregulation of modulation within the locus ceruleus- norepinephrine and sympathetic nervous systems and hypothalamic-hypopituitary-adrenocortical axis.The aim of this case report is to show the effects of combined psychopharmacotherapy (using vortioxetine, pregabalin and alprazolam) and psychotherapy (group analysis-oriented psychotherapy and individual cognitive behavioural approach) in female patient with generalized anxiety disorder and depression comorbidity. Results showed a significant alleviation of anxiety and depression measured by the score scales HAM-D-17 and GAD7. In conclusion, in this female patient a combination of psychopharmacotherapy and psychotherapy also led to achieving functional recovery by withdrawing the gastrointestinal symptoms that mimicked the presence of celiac disease.
