Isolated right-ventricular hypertrophic cardiomyopathy with midcavitary obstruction and high intracavitary pressure gradient
Hypertrophic cardiomyopathy is usually presented as a disease of the left ventricle. However, with the left ventricular hypertrophy, the right ventricle may also be affected. In rare cases, right ventricular hypertrophy appears to be dominant, and can be presented in an obstructive and non-obstructive variant. Isolated right ventricular hypertrophy seems to be very rare. This paper presents an extremely rare case of isolated right ventricular hypertrophy in 39-year-old completely asymptomatic female patient. At the age of 4 she had cardiothoracic surgery for congenital pulmonary stenosis (valvulotomy), and at the age of 15 she suffered from rheumatic fever. Severe hypertrophy involved the apical half of the right ventricular free wall and the interventricular septum. Right ventricle took a shape that looked like a sand timer, whereas narrowing between its apical and basal part caused systolic pressure gradient of 186 mmHg but also diastolic gradient of 2.7 mmHg. The diagnosis was obtained by echocardiography and confirmed by magnetic resonance imaging and right-sided cardiac catheterization.
