Duration of diabetes mellitus and the occurrence of cerebrovascular disease

Large population-based epidemiological studies have demonstrated that diabetes mellitus (DM) is a major risk factor for cerebrovascular disease (CVD). The aim of this study was to examine the influence of duration of type 2 DM on the cerebrovascular atherosclerotic changes. Our study group consisted of 60 patients (pts) with type 2 DM divided into two groups dependent on duration of disease (<10 yrs and >10 yrs). Color dopler flow imaging (CDFI) of carotid and vertebral arteries and transcranial dopler ultrasonography (TCD) were performed. In the group of pts with DM duration under 10 yrs intima-media tickness (IMT) was found in 68%, mild carotid stenosis in 25% and moderate carotid stenosis in 7% of cases. In the group with duration of disease over 10 yrs we found IMT in 31%, mild stenosis in 35%, moderate stenosis in 31% and carotid occlusion in 3%. Spearman correlation showed statistically significant difference between two groups (p<0.01). In the first group TCD analysis showed signs of microangiopathy in 87% and macroangiopathy in 13% pts. In the second group microangiopathy was found in 48% and macroangiopathy in 52%. Chi square test shows statistically signifficant difference between the two groups (p<0.05). The study confirm type 2 DM as risk factor for CVD and shows that atherosclerotic changes of cerebral vasculature are intensifying with longer duration of disease.
