Epileptic Seizures in Patients With Ischemic Stroke
Aim of the study was to evaluate the relationship between ischemic stroke (IS) and epileptic seizures(ES).We analyzed clinical presentation of seizures, their relation to teritorial localization of IS, comorbid risk factors for IS, and lethal outcome. The retrospective study encompassed 1504 inpatients who had been treated for IS during 20 months at our department. 45 of them (3%) had ES (28 males and 17 females, mean age 69.5 years). 28 patients had ES as an initial symptom of acute stroke, and 17 as a symptom due to the prolongued cerebral ischemia. Most of the patients had a partial ES (15 motor, 3 sensory, 2 sensomotor, 3 complex, 3 adversive, and one partial epileptic status), and 16 patients had a generalized form of ES.Hypertension and alcoholism were most frequent comorbid risk factors with IS with generalized ES, while heart diseases, followed by hypertension, were most common comorbid risk factors for IS with focal ES. Large teritorial infarcts localized in the anterior portion of brain were more related to ES then other types of IS.The short- term lethal outcome rate for IS patients with ES was 20%, higher than within the whole group of IS patients (14.3%). Epileptic seizures, although not a common clinical feature in IS, may influence together with frequent comorbid risk factors (most notably hypertension, alcocholism and heart diseases) the unfavorable short-term outcome of ischemic stroke patients.
