Irinotecan-induced leucopenia in rats: effects of Δ9-tetrahydrocannabinol co-treatment

There is considerable interest for natural products that contain cannabinoids in combination with conventional antineoplastic therapy.However, cannabinoid-drug interaction may be associated with the risk of clinically relevant toxicities. Irinotecan (CPT-11) is one of the most important anticancer drugs used to treat different solid tumors and metastatic colorectal cancer but its use is associated with clinically important toxic effects particulary leucopenia and delayed diarrhea. Here, we report on the acute clinical effects of irinotecan + Δ9-THC on Wistar rats. Serial blood samples were obtained up to seven days after dosing and were analyzed for complete blood count and inflammatory markers (CRP and TNF-α). Both irinotecan and irinotecan + Δ9-THC caused moderate leucopenia but a greater decrease in leukocyte count (10exp9/L) was observed in the irinotecan + Δ9-THC treated group (1.4 ± 0.3) compared to the irinotecan-treated alone (2.7 ± 0.8). This study suggests that a single irinotecan dose of 100 mg/kg b.w. was sufficient to induce moderate leucopenia in Wistar rats without diarrheal symptoms and without an increase in circulating pro-inflammatory mediators. Since concomitant Δ9-THC administration to irinotecan.treated rats might produce higher immunosuppression than irinotecan alone, further studies are needed to investigate the influence of medicinal cannabinoids on hematological toxicity.
