Important features of frontal sinuses for the eybroww keyhole approach
Background and Purpose ; We assessed anatomical aspects or me frontal sinuses important for minimally invasive procedures in microneurosurgery. The influence of clinical anatomy of frontal sinuses on the eyebrow keyhole approach was analyzed in particular. Material and Methods: Variations and relations of frontal sinuses were explored on 50 anatomical specimens, 100 skull x-rays and 200 intraoperative findings with special reference to lateral and posterior extensions of the frontal sinus pneumatization. Results: Extensions of the frontal sinuses impeded craniotomy in 44.2% of cases. Precisely in 35:8% of cases, craniotomy was hampered exclusively by the wide lateral extension of the frontal sinus, while in 8.4% of cases the keyhole approach was additionally hampered by pronounced posterior sinus extension causing elevations of the orbital roof into which the sinus extends. According to the above features, frontal sinuses were classified into four types. A detailed morphometric and statistical study of the lateral frontal sinuses extensions on anatomical specimens was carried out. Conclusion: Comparison of the data provided useful information on the anatomical features substantial for proper planning of the neurosurgical eyebrow keyhole approach in the anterior cranial fossa.
