Prophylaxis of ophthalmia neonatorum in Croatia - a survey of the current practice
Aim: Ophthalmia neonatorum (ON) or neonatal conjunctivitis is an acute mucopurulent infection of the conjunctivae occurring within the first 28 days of life. In order to prevent ON different number of prophylactic antibiotic or antiseptic agents have been used. The aim of this study was to analyze the current practice of ON prophylaxis in Croatia. Methods: A non-experimental quantitative empirical approach was used. The data was collected by means of predesigned questionnaires. Questionnaires were sent to all of the 32 maternity hospitals in Croatia with a total of approximately 40, 000 births a year. The overall response rate of participation was 100%. Results: ON prophylaxis is administrated to all newborns in 75% (24/32) of Croatian maternity hospitals. Eleven (45.83%) of them perform these procedures in the first hour after birth and 13 (54.17%) in the period from one to three hours after birth with the prevailing medicine being Tobramycin (83.32%). Other applied substances were Povidone-iodine (8.34%), Erythromycin (4.17%) and Silver nitrate (4.17%). In 25% of maternity hospital prophylaxis of conjunctivitis is not used routinely however antibiotic treatment with Tobramycin in case of symptoms of conjunctivitis was applied. Conclusion: The rationale for prophylaxis and the substances used in Croatia show heterogeneity. Four prophylactic agents are generally used, two antiseptics (Povidone-iodine, Silver nitrate) and two antibiotics (Tobramycin, Erythromycin). In most cases of prophylaxis (83.31%) and all cases of treatment Tobramycin is applied. Considering different approaches to prophylaxis it would be beneficial to determine a nation-wide agreement for ON prophylaxis use in Croatia.
