Salivary immunoglobulin A and G subclasses in oral mucosal diseases.

The purpose of this study was to elucidate local immunologic deficiency of salivary immunoglobulin IgG and IgA subclasses in patients with oral musocal diseases. We have measured salivary levels of IgG1, IgG2, IgG3, IgG4, IgA1 and IgA2 by using ELISA and RID in resting whole saliva of 31 patients with acute RAU, in 11 patients with chronic hyperplastic candidiasis, in 12 patients with Sjögren's syndrome, in 6 patients with oral lichen planus, and compared with 20 age and sex matched controls. Statistical analysis was performed by Mann-Whitney and Wilcoxon tests. Significantly higher levels of salivary IgG1, IgG2, IgG3, IgG4 and IgA2 were found in patients with acute RAU when compared to healthy controls. The level of salivary IgA1 differed insignificantly between acute RAU and controls. Significantly elevated levels of IgG1, IgG2, IgG3, IgG4 and IgA1 were found in patients with chronic hyperplastic candidiasis when compared to the control group. No significant differences between levels of salivary IgA2 in patients with chronic hyperplastic candidiasis and controls were found. Significantly elevated values in patients suffering from Sjögren's syndrome in comparison to the controls were found in salivary IgG1, IgG3, IgG4 whereas no significant differences between these groups in salivary IgG2, IgA1 and IgA2 could be detected. In patients with oral lichen planus significantly elevated values of salivary IgG1, IgG2, IgG3, IgG4 and IgA1 and IgA2 when compared to the control group were found. We can conclude that subclasses of salivary immunoglobulins IgG and IgA play a role in the oral mucosal diseases.
