Importance of Drug-induced Sleep Endoscopy (DISE) and VOTE Score system in OSAS Diagnostics: Our expirience
When surgery for OSAS is considered, assessment of the sites of obstruction is of great importance for surgical success. Drug- induced sleep endoscopy (DISE) is unique in assessing the cause of the apnea and/or snoring by pharmacologically artificially inducing the patient into a light sleep ; flexible endoscopy of the upper airway is then performed. Our experience in DISE findings was reported using Propofol for sleep induction and the VOTE classification system. Site, degree of airway narrowing, and configuration of obstruction were reported. The four most common obstruction sites are evaluated when using the VOTE classification: the velum, oropharynx (including tonsils), base of tongue, and epiglottis. Airway closure can occur with collapse in an antero-posterior, lateral, or concentric configuration. DISE is safe and easy-to-perform dynamic technique that visualizes the anatomic sites of snoring or apneas and guides the design of a tailor-made treatment plan in individual cases. The VOTE classification, an easily applicable grading system, facilitates data collection and DISE findings reports.
