Sažetak
In recent years, there is a trend of increase in the number of leishmaniasis cases in European countries which is probably associated with increasing travels and immigration. In Croatia, cutaneous leishmaniasis has been reported very rarely, usually in the southernmost part of the country, or as an imported infection in patients returning from travels to one of the endemic countries abroad. We will present three cases of cutaneous leishmaniasis diagnosed at our Department in the year of 2017. The patients were not related one to another, but all three had a history of recent travel to the Littoral or Northern Dalmatian coast. Diagnosis was based on the histopathological finding of amastigotes in incisional biopsies. In addition, molecular testing by polymerase chain reaction - restriction fragment length polymorphism analysis of ITS1 for Leishmania species identification confirmed infection by Leishmania infantum in all three cases. Patients were treated with prolonged course of oral fluconazole, and additionally with topical paromomycin in one patient, resulting in complete clinical remission. The presented cases indicate that there is a need for better awareness of this vector-borne infection, as it may pass undiagnosed or the diagnosis may be delayed, causing greater impact on the patient in terms of scarring or propagation of the disease. Public health measures and better control of canine leishmaniasis should also take place since domestic dogs serve as main reservoir host.
