Expression of LIGHT and its receptors in lichen planus

Introduction: Lichen planus is a cell-mediated immune disease of unknown origin, characterized by dermal mononuclear cell inflammatory infiltrate and damage of the epidermal basal layer. The purpose of our study was to investigate whether this damage might be mediated by the TNF superfamily member 14 (LIGHT/TNFSF14) and its receptors receptors lymphotoxin beta receptor (LTbR) and TNF receptor superfamily member 14 (HVEM/TNFRSF14). Material and Methods: Immunohistochemistry for LIGHT, LTbR and HVEM was performed on formalin-fixed paraffin-embedded tissue samples obtained from lesional skin of patients with lichen planus and samples of healthy skin (n=10 per group). Results: Immunohistochemical staining for LIGHT was detected both in the epidermis and the dermal infiltrate of lichen planus biopsies. When compared to the relatively weak staining observed in the epidermis of healthy skin, the epidermal staining was enhanced in biopsies of lichen planus. Small proportion of the cells of the lichenoid infiltrate also stained for LIGHT. The epidermal expression of LTbR was weaker in lichen planus compared to healthy skin, but it was present among many cells of the inflammatory lichenoid infiltrate. Staining for HVEM was also observed among numerous cells of the infiltrate as well as in the basal layer of the epidermis. Discussion: The observed pattern of staining for LIGHT receptors LTbR and HVEM indicates a possible role of LIGHT-mediated signaling in the pathogenesis of lichen planus. This warrants further investigations of the potential role of LIGHT, LTbR and HVEM in this and other inflammatory diseases characterized by lichenoid infiltrate and apoptotic death of keratinocytes, such as lupus erythematosus, erythema multiforme and others.
