Condilectomy and orthognathic surgery approach in a patient with facial asymmetry
AIMS: To show the results of orthodontic treatment and simultaneous surgical procedures, condylectomy, bimaxillary osteotomy and wing osteotomy of the chin, for treatment of facial asymmetry and malocclusion. SUBJECT AND METHOD: A 34-year-old woman referred for facial asymmetry with a chin deviation to the right side and malocclusion. Intraorally, the midline of the mandibular teeth was deviated to the right side by 4 mm. She showed an anterior crossbite and posterior crossbite on the right side and canting of the occlusal plane on the right up to 4 mm. Radiographic examination showed an increased left ramus height of 6 mm. After orthodontic preparation to harmonize the transverse relationships and level the dental arches a condylectomy with simultaneous bimaxillary orthognathic surgery and a wing osteotomy was performed. Post-operative orthodontic treatment was carried out for fine adjustment of the occlusion to achieve maximum intercuspation. RESULTS: After one year a cone-beam computed tomography image showed the condyle in a bone remodelling state. CONCLUSION: Orthodontics and combined simultaneous surgery can bring excellent functional and aesthetic improvement in a more comfortable and shorter time.
