One-week therapy (omeprazole, amoxycillin, azithromycin) for cure of Helicobacter pylori infection in duodenal ulcer disease

Background: Previous studies have show that one-week triple therapy consisting of omeprazole, clarithromycin and amoxycillin may cure Helicobacter pylori infectionin the vastmajority of patients. Aim: Our study was designed to test the hypothesis that a triple therapy with omeprazole, amoxycillin and azitromycin cures the infection in 80% of duodenal ulcer patients infected with H. pylori. Methods: In an open four-centre study, 60 patients with duodenal ulcer and H. pylori were treated with omeprazole 2 x 40mg/day, amoxycillin 2x1000 mg/day for 7days and azithromycin 500 mg/day for 6days. The anti-H. pylori therapy was followed by a 3-weej course withomeprazole 40 mg/day and 20 mg/day (last 2 weeks) before breakfast. We assessed H. pylori infection before treatment and 4 weeks after cessation of medication by a rapid urease test and histology after Giemsa stain. Results: 60 patients (40 males, mean age 45, 7 years)entered the trial During the study 4 patients dropped out, one because of allergy to pencillin and 3 patients were protocal violators. 26 patients reported 48 adverse events that were mostly mild to moderate. H. pylori infection was cure din 84% (47 of 56) patients who completed the trial according to the protocol and in 80% (48 of 60) patients included in the trial. 4 weeks af the last administration of study drugs all ulcers had healed. Conclusion ; One-week triple therapy consisting of omrprazole, amoxycillin and azithromycin is a simple and effective approach to the cure of H. pylori infection in patients with duodental ulcer.
