Problem At the materno-fetal interface the fetoplacental unit (FPU) tissues come into direct contact with maternal decidua which is haevily infiltrated with maternal cytolytic lymphocytes of both antigen-specific and MHC restricted killing and antigen nonspecific and MHC nonrestricted killing. Decidual NK cells (CD3-CD56 bright CD16-) with a high content of cytolytic mediator perforin (P bright) are the prevalent cell population. Th1 type cytokines are known as activators of cytolytic machinery. The aim was to investigate the role of Th1 cytokines in P induction, upregulation of P expression and cytotoxic activity of cytotoxic lymphocytes at the interface. Methods First trimester pregnancy decidual and peripheral blood lymphocytes were obtained from elective termination of pregnancy. P-protein was detected by flow cytometry, immunocytochemistry and immunohistochemistry, mRNA for P by RT-PCR and cytolytic potential of cells with PKH-26 (orange)-labeled K562 target cells and flow cytometry. Results Decidual adherent cells and their supernatants are important in the regulation of P expression at high levels and this could be blocked by antibodies anti IL-10 and IL-15. P expression and cytolytic activity of decidual lymphocytes (DL) can be upregulated by IL-2, IL-12, IL-15 (high concentrations) and IL-18 and combinations of IL-12 + IL-15, IL-12+IL-18, IL-2+IL-15, IL-2+IL-12 and IL-15+IL-18. The distribution of IL-15+ cells was similar as distribution of macrophages, and IL-18+ cells show the distribution as GZCT4+ cells (marker for trophoblastic cells), as well. Conclusion The cytolytic machinery of decidual lymphocytes is maximally primed for function (killing) during pregnancy but is simultaneously very efficiently controlled to prevent accidental activation that could result in the loss of FPU. Infection and abnormal immune activation (Th1 response) are probably effective trigger mechanisms for release of cytolytic potential.
