CLINICAL AND DIAGNOSTIC FEATURES OF PSORIASIS AND PSORIATIC ARTHRITIS

Psoriasis is a systemic chronic immune-inflammatory-mediated disease which affects 2-3% of worldwide population. Although its systemic nature very often remains unrecognized, the inflammatory processes in so-called “psoriatic disease” may be associated with the development of various comorbidities which may have a significant impact on the patient's health and wellbeing. Psoriatic arthritis (PsA) is a major comorbidity of psoriasis which significantly impairs physical function and as well the quality of life, and affects from up to 40% psoriatic patients. Psoriasis appears to precede the onset of psoriatic arthritis in 60-80% of patients and in as many as 15-20% of patients, arthritis appears before the psoriasis. In some cases, arthritis and psoriasis develop simultaneously. Patients with PsA suffer from a number of comorbidities, many of which are precipitated by persistent tissue inflammation, such as cardiovascular disease, diabetes, the metabolic syndrome, and depression. Therefore, dermatologists are in a unique position to identify patients at risk for psoriatic arthritis before irreversible joint damage occurs. The therapy of the whole spectrum of psoriatic disease has been dramatically changed in the last two decades thanks to the introduction of a plethora of new emerging biologic and small molecules therapeutics ; these new therapeutic options are equally in use in dermatology and rheumatology. In this presentation, we review main clinical and diagnostic features of psoriasis and psoriatic arthritis, as well as available therapeutic options. It is very important to point out the necessity of close cooperation and team work of dermatologists and rheumatologists in a therapeutic approach to psoriasis and psoriatic arthitis.
