HLA-B/HLA-C Region is associated with Familial Juvenile Onset Psoriasis in Croatian Patients

To evaluate the nature of the HLA association with psoriasis, HLA-haplotypes of 58 unrelated patients with type I psoriasis (early onset, <40 years, positive family history) were investigated by high-resolution PCR-based typing with sequence specific primers for HLA class I and II. Volunteer blood donors were typed as a controls (N=78). Results showed 34, 5% of patients to be positive for HLA-B/-C region of extended haplotype EH-57.1(Cw*0602-B*57) previously described as the most frequent psoriasis associated haplotype, and 27, 6% of patients demonstrated Cw*0602-B*13 haplotype, both highly overrepresented (p<0.001) compared to only 2.1% and 1.8% in control, respectively. Further analysis of individual HLA alleles revealed that HLA class I antigens were to a great extent associate with type I psoriasis: Cw*0602 with a gene frequency of 0.345 in patients vs. 0.0658 in control (p<0.001, RR=14.6), B*57 0.172 vs. 0.0116 (p<0.001, RR=19.4) and B*13 0.138 vs. 0.011 (p<0.001, RR=14.1). Pedigree analysis of the four affected families over two to three generations, revealed cosegregation of the disease with HLA-B/-C region. In our patient group, there was evidence of a prental sex effect ; 53.7% of patients had an affected father, while 24.3% patients had an affected mother (p=0.06).
