Unusual adverse event following radiofrequency genicular nerves ablation (RFA)
Introduction: According to the recent studies chronic knee pain after total knee replacement is estimated to be as high as 40 %. High percentage of patients also have an increase of analgesics drugs consumption after the operation. Radiofrequency ablation of the genicular nerves, as a part of the interventional analgesic treatment, is supposed to be a safe and effective procedure with rare complications. Case report: We report a 78 years old female patient with chronic left knee pain after total knee replacement who was scheduled for RFA (sensory stimulation 50 Hz, 1msec at less than 0, 6 Volts ; motor stimulation 2 Hz, 1 msec at 2 Volts ; following RFA with temperature of 70⁰ C during 90 sec). Several hours after the procedure she developed an oedema and redness of the left leg. She came to the emergency departement the third day after the RFA and laboratory analysis revealed a significant elevation of CRP, creatin kinase, liver enzymes as well as urea and creatinin. X-ray scan did not point to peri- prosthetic infection. Intravenous clindamycin and ceftriaxone started. The seventh day after the procedure bullae on the skin of the lower leg occured similar to skin burns. Elevated laboratory values by the tenth day declined and came back to normal, but the skin lessions were healing very slowly. Conclusions: We cannot conclude with certainty weather the allergic reaction with secundary infection of the tissues or heat induced injury caused by thermal energy are resposible for this adverse event.
