Bone expansion and bone condensation - alternative osteotomy tecnhiques for compromised alveolar ridge

Unfavorable volume of the alveolar bone is a special clinical challenge in dental implantology. Bone resorption, with a consequential width and height reduction of the alveolar ridge, is a frequent problem. In the cases of horizontal bone reduction, bone expansion and bone condensation, prior to dental implant placement, make additional augmentation procedures unnecessary. D-shaped bone expanders, with an increasing diameter, push the bone laterally by separating the labial and palatal bone lamellae. We present the advantages and disadvantages of bone expansion and condensation in gaining the horizontal dimension of the maxillary alveolar ridge. We used bone condensers after the bone expansion, which, due to the rounded profile, adjusted the bone cavity for the desired implant diameter. Afterwards implants were placed in the soft bone bed, without requiring the use of tap. Four patients underwent bone expansion and bone condensation in the maxillary alveolar ridge. One or more dental implants were placed (Ankylos ®, Sirona Dentsply, Germany) and a particulated xenograft (Biooss®, Geistlichz, Germany) was used. After a three-year follow up, satisfactory functionality and esthetics were observed in all four cases. Bone expansion had been shown unsuitable for increasing the vertical bone volume. Also, poorly structured bone will show low resistance during pilot drilling. Thereby, it should be condensed by a bone condenser, which will expand the bone to the adequate diameter of the planned implant. After bone expansion and bone condensation, the healing phase should be extended to six months. Functional stimulation of the implants will limit the resorption of the resisted bone.
