MODIFIED HADLOCK 2 FORMULA FOR ULTRASOUND ESTIMATION OF NEWBORNS WEIGHT IN GDM OBESE WOMEN

Summary. Introduction. The frequency of GDM is increasing, especially after the HAPO study recommendations for glucose values to determine gestational diabetes mellitus (GDM). Wlee known, GDM is a risk factor that carries adverse perinatal outcome such as macrosomia, dystocia, operative delivery, neonatal complications and many other. The ultrasound estimation of newborns weight is of great importance for clinical decision making. Objective. The aim of the study was to explore the influence of weight gain among obese women with GDM on ultrasound newborn weight estimation using modified Hadlock 2 formula. Methods. The study included 75 women with singleton pregnancies, they all were obese and had weight gain greater than recommended. Ultrasound was performed during pregnancy and within three days before delivery. The two formulas – Hadlock 2 and modified one were compared. Results. The estimated newborns weight estimated by modified formula had significantly less difference compared to newborns weight in comparison with the weight estimated by Hadlock 2 formula (P < 0.001). The modified formula had less false negative results concerning macrosomia (P < 0.001) compared to Hadlock 2 formula. Modified Hadlock 2 formula for the estimation of newborns weight was more precise for women with GDM and over-than-recommended weight gain compared to Hadlock 2 (P < 0.001). Discussion. The information of precise newborn weight is of great importance for delivery management. The usage of Hadlock 2 formula might be inadequately among women with GDM and large weight gain. Conclusion. To avoid under- and overestimation of newborn weight the modification of existing formulas have to be considered.
