MULTIFACTORIAL APPROACH-THE KEY TO SUCCESS IN THE TREATMENT OF TYPE 2 DIABETES

INTRODUCTION: Over the last few decades, rates of type 2 diabetes (T2D) have increased markedly in parallel with a global rise in obesity. Management of T2D focuses on lifestyle interventions, maintaining blood glucose levels in the normal range and lowering other cardiovascular risk factors. A diabetic diet that promotes weight loss and exercise are the foundations of diabetic care. Recently emerging anti-diabetes drug classes, such as GLP-1 agonists, are more “weight-friendly” than previously available treatments. CASE REPORT: We present the case of a 63 year old woman diagnosed with T2D, extreme obesity with secondary alveolar hypoventilation (Sy Pickwick) and arterial hypertension.At the time of initial presentation to the outpatient diabetic clinic her weight was 118 kg, height 151 cm, BMI 52 kg/m2. Her laboratory findings were: HbA1c 9.5%, total cholesterol 5.1 mmol/L, HDL 1.1 mmol/L, LDL 3.4 mmol/L and triglycerides 1.3 mmol/L. In addition to her standard therapy exenatide once-weekly (Bydureon®) was recommended. The patient was also referred to diabetes education classes. At the follow-up visit 4 months later, patient was pleased that she had lost 20 kg. Her HbA1c had greatly improved to 5.7%. By sticking to a healthy eating plan and a healthy lifestyle, the patient had lost additional 43kg within a year and her laboratory findings improved even more: HbA1c 5.4%, cholesterol 4.0 mmol/L, LDL 2.6, HDL 1.0 mmol/L, triglycerides 0.8 mmol/L. DISCUSSION: The case presented here supplements the available data on the utility of GLP-1 receptor agonist therapy in combination with healthy lifestyle for achieving and maintaining good glycemic control and weight loss. Diabetes education programs aiming to promote healthy eating habits and provide patients with the tools they need to control glycemia and avoid long term complications, are of great importance for successful diabetes management.
