GLEASON SYSTEM GRADING OF PROSTATIC ADENOCARCINOMA: A COMPARISON OF OLD AND NEW CLASSIFICATION
Introduction: Despite the revisions Gleason score as a system of prostate cancer grading is insufficient prognostic and therapeutic factor. The recent introduction of modified Gleason classification with prognostic groups better reflect the nature of the disease. Aim: To verify the value of the new classification in groups of patients with different prognostic parameters. Materials and methods: 146 patients with acinar type of prostate adenocarcinoma treated with radical prostatectomy with clinical and histopathological data were seleted from the files of the Urology and Pathology Department of the Clinical Hospital Center Rijeka. Known prognostic parameters, disease stage and preoperative PSA were analyzed. The outcome was based on the data of biochemical recurrence of the disease. We compared the results of the old and the new Gleason classification using Cramer's V measure of association between two nominal variables and the nonparametric Mann Whitney test. Results:There was a better association between the biochemical recurrence and prognostic groups (Cramér's V = 0.38 )compared to the old grading by Gleason score (Cramér's V = 0.25). There were no differences between the new and the old system of grading regarding the stage of the disease(Cramér's V = 0.42 ; Cramér's V = 0.43 respectively). Mann-Whitney test showed signifant difference between the prognostic groups regarding preoperative PSA values (p = 0.01), while there was no significant difference when we used old Gleason scoring (p = 0.55). Conclusion:The modified Gleason classification using prognostic groups better correlates with disease outcome and preoperative PSA values than the old Gleason scoring system.
