Sacroiliac dysfunction in pregnancy-a conservative approach

Objective: Sacroiliac dysfunction is major pain syndrome in pregnancy. It is caused by specific structure and function of the sacroiliac joints during pregnancy with incidence up to 76.6%. Aim: to determine the effectiveness of exercises in order to stabilize the pelvis and compare it to support belt, both used to improve sacroiliac dysfunction during pregnancy. Methods: prospective study of 80 pregnant women in each trimester (total, n =240). Exclusion criteria were surgical procedures in the area of the spine and pelvic as well as and symptomatic appearance of sacroiliac dysfunction before pregnancy. Study group has conducted exercises to stabilize the pelvis three times a week for 45 minutes during two weeks period under the supervision of a physiotherapist. The control group was wearing a support belt every day during daily activities for two weeks. Numeric Pain Rating Scale (NPR scale) was used to assess the pain intensity while Quebec scale was used for assessment of the degree of disability in everyday activities. Results: statistical significance was found (p<0.05) in assessment of the increase of pain by all trimesters, with significantly increased disability while performing everyday activities in both groups (p<0.05). Statistically significant difference was also found in pain intensity in both groups, but degree of disability was significantlu better in exercise compared to belt group (p<0.001). Conclusion: our results suggest superiority of exercises used to stabilize the pelvis during ongoing pregnancy over simple support belt for sacroiliac dysfunction in pregnancy. Also, the results of the study suggest an important role of physiotherapists in a conservative approach to treating sacroiliac dysfunction in pregnancy.
