CLINICAL SIGNIFICANCE OF DRUG METABOLITE PROFILING
General biotransformation mechanisms of xenobiotics in humans have been largely elucidated during the last three decades. Until recently, this accumulated knowledge of biotransformation mechanisms have not been significantly utilized in the clinical settings. Recently, these biotransformation pathways stepped outside of boundaries of academic importance, finding their utility in the therapeutic drug monitoring and forensic medicine. Metabolite profiling has also gained importance and utility as a tool used in pharmacovigilance and as a prognostic marker for the therapeutic outcomes. As is the case with the parent compounds, metabolite profiling is used for monitoring of drugs characterized by the narrow therapeutic range: quantitative determination of immunosuppressants, antiepileptics, psychoactive and anticancer drug metabolites has already entered the routine clinical application. A critical overview of the literature and case study evidence supporting the metabolite profiling applications in the clinical settings will be presented.
