Failure identification and analysis is a systematic investigative procedure using the scientific method to identify the causes of a failure and propose the solution. Analysis of failures should be an integral part of design and manufacturing processes. Besides suggesting incremental improvements for the future designs, proper failure analysis can also facilitate prevention of catastrophic failures. The systematic use of evidence-based analytical procedures by engineers increases their capability to identify the probable causes of incidents involving engineering structures and offer innovative solutions. Such procedures also provide a reference for design implementation by offering a perspective that considers factors other than the conventional concerns of quality, cost, and duration of work. Therefore, failure identification and analysis (FIA) methodology reduce the difficulties caused by risks of future damage or the follow up management of incidents. Regarding the overall engineering project, existing failure reports of similar engineering accidents can be incorporated into the planning and design stage during the the project lifecycle. A design procedure considering failures of the previous system design can be formulated by developing in-depth understanding of the system from both success and failure point of view. Any modification in the design either by addition of new subsystems based on the new technological principles or by modification of existing solution can affect the reliability of the system. In addition to limiting risk in future projects, using existing forensic data also improves engineering performance and reduces risk-related costs. Although several techniques exist to address such type of problems, and usually are used as an individual tool (RCA, DSM, TRIZ), each method has its limitations when used in isolation. A standalone approach may jeopardise the performance issues because the integral understanding and modelling of the complex situation is not captured. Therefore, a recent trend is to combine different techniques together in the one framework, and here presented FIA methodology is tailored specifically for the ALSTOM.
