The Incidence and Distribution of Cystic Lesions of the Knee Joint
AIM: To analyze the type and distribution of cystic lesions of the knee, and their incidence depending on age. METHODS: The study included 300 patients with cystic lesions of the knee, found on MRI. The type and distribution of cystic lesions and their incidence in relation to age of patients were analyzed. Patients were divided into three age groups: 18-35, 36-55, 56-70 years. In each group, 100 patients were randomly included. Exclusion criteria: previous knee surgery ; inflammatory disease. RESULTS: Age group 18-35 (median 28.5): dominantly distended bursae and synovial cysts in 52% of patients ; subchondral cysts (21% of patients) ; meniscal cysts (16% of patients) ; ganglion cysts (12% of patients) ; vascular malformations (4% of patients). Age group 36-55 (median 44.0): almost equally subchondral cysts in 48% of patients, and distended bursae and synovial cysts in 44% of patients ; meniscal cysts (18% of patients) ; ganglion cysts (14% of patients) ; rarely vascular malformations (2% of patients). Age group 56-70 (median 61.5): dominantly subchondral cysts in 76% of patients ; distended bursae and synovial cysts (42% of patients) ; ganglion cysts (11% of patients) ; meniscal cysts (10% of patients) ; vascular malformations were not found. CONCLUSION: Subchondral bone cysts, as the overall most common cystic lesions of the knee, are usually formed in regions of maximum load. Osteoarthritis, especially in elderly patients, is a main reason for the dominance of subchondral cysts. In the young and middle-aged patients distended bursae and synovial cysts are dominant cystic lesions, most likely due to overuse and traumatic injuries.
