Primary extradural meningiomas of the scull: a report of 11 cases

Purpose: A study was performed to evaluate the radiological findings of 11 primary extradural meningiomas (PEM) of scull. Methods: Retrospective analysis of 11 patients (8 females and 3 males) with intraosseous meningiomas found during four-year period. We analyzed computed tomography (CT) and magnetic resonance imaging (MRI) findings, demographic features, site and morphological type of the tumor. In our study PEM was defined as a tumor with the largest component inside the bone (primary intraosseous meningioma) while dural invasion was not exclusive criteria. Results: The age of male patients ranged from 37 to 62 years (median 53), and female patients from 42 to 75 years (median 58). Plain radiographs were taken in one, CT in seven and MRI in ten patients. Frontotemporal region was the most common tumor location (91%). Extracalvarian orbital soft-tissue component had 6 patients (55%). All patients had osteoblastic subtype of PEM. One patient (9%) had type 2B (purely calvarial scull base tumor), 6 patients (55%) type 3B (calvarial with extracalvarial extension on scull base) and 4 patients (36%) type 3C (calvarial with extracalvarial extension on convexity). Conclusion: CT with bone window is necessary in detection of the tumor and its intraosseous extension. Evaluation of soft tissue component of the lesion is better with MRI. The differential diagnosis of the osteoblasic subtype of primary intraosseous meningioma includes: fibrous dysplasia, osteoma, osteoblastic metastasis, osteosarcoma and Paget disease.
