Evidence based Information Fusion for Enabling Personalized Treatment of Diabetic Patients

We have developed an e-platform aiming to help persons with diabetes to direct self-management and at the same time, to enable healthcare professionals to provide management of diabetes based on accurate, measured data on blood glucose level and physical activity. The motivation for developing the e-platform is twofold: our research has shown that data presented by the persons with diabetes in the so called diabetic diary is often inaccurate and missing, while on the other hand, handling of several dozens of different blood glucose measurement (glucometers) devices presents a large problem to health professionals due to non-standardized interfaces (both physical layer and protocols) and lack of a unique platform for storing and visualization of those data. The other parameter considered to be important in management of diabetes, physical activity, was often acquired by questioners which gave only descriptive and superficial information on activity. In our system, the information on measured blood glucose levels stored in glucometer memory is transferred to the e-platform by connecting the glucometer to a custom developed interface which enables transfer of the data to the e-platform by mobile phones. The same interface device contains inertial and magnetic sensors for measurement of physical activity including guidance for endurance and strength exercising. Data management and visualization software, and the database for central storage of the data are integral parts of the e-platform.
