Hip Osteonecrosis
Osteonecrosis is now a commonly recognized disorder with significant morbidity. Diagnosis of this disorder has increased because of improved technology and increased awareness. The end stage of the process is severe destruction of the femoral head with resultant degeneration of the hip joint. If left untreated, the disease progresses in 80 % of patients and eventually requires total hip arthrosplasty. In many patients, even early identification and intervention do not alter the result. Unfortunately, patients who are affected with osteonecrosis are young, usually in the third to sixth decades of life. Imaging of osteonecrosis is frequently diagnostic with a serpentine rim of sclerosis on radiographs, increased uptake in early disease at bone scintigraphy, and specific findings at MR. These radiologic features correspond to the underlying pathology of osseous response to wall off the osteonecrotic process and attempts at repair with vascularized granulation tissue at the reactive interface. Radiography is relatively insensitive for the detection of the early stage of AVN. Magnetic resonance imaging (MRI)is highly sensitive in depicting early AVN and is method of choice for accurate diagnosis and staging of the disease with reliable information about severity, prognosis and treatment response.
