Eosinophilic erythema annulare

Eosinophilic annular erythema (EAE) belongs to a group of figurate erythemas with a prominent tissue eosinophilia (there are only few case reports described in the literature), and is characterized clinically by the appearance of polycyclic lesions and annular plaques. We report a case of a 59 year old male patient who presented with one month history of itchy, polycyclic and annular lesions localized on the trunk, gluteal region and extremities. There was no medical history of drug intake. Extensive laboratory work up including serological infectious disease testing was done and it was within normal range. Skin biopsy showed a dense perivascular lymphocytic infiltrate with abundant eosinophils interstitially and within the subcutaneous fat. The aforementioned histological characteristics were compatible with the diagnosis of Erythema annulare centrifugum. High potency local steroids along with antihistamines were prescribed but without adequate therapeutic result. The patient was then given orally 40mg of prednisone but relapsed soon after the discontinuation of the treatment. Chloroquine was started in dose of 4mg per kg daily for 10 days, then 250 mg daily for 10 months, resulting in a complete clearance of all the lesions. The patient is well and disease free for 15 months now. According to some authors EAE is a clinical subtype of Wells syndrome presenting with an annular or figurate pattern. The etiology of EAE is still unknown and further reports may permit a better understanding of the prompt response to antimalarials.
