Validacija kratke procjene kognitivnih sposobnosti kod bolesnika na rehabilitaciji nakon traumatske ozljede mozga
Introduction: Early assessment of cognitive functions after traumatic brain injury (TBI) is required for the planning of rehabilitation, follow-up, and for providing information to patients and their families. Mainly due to the duration of neuropsychological testing, a number of patients can not be assessed with standardized tests. Objective: To assess the impairments of cognitive functions using a short battery of tests, and to examine diagnostic and predictive validity of a short cognitive test battery. Methods: In 50 patients on the hospital rehabilitation after a severe traumatic brain injury, we applied tests of orientation, picture recognition, logical memory, apraxia, visual perception, neglect, picture naming and verbal fluency. Tests were applied at the beginning of the rehabilitation, an average of 2.5 months after a severe TBI. The results of subtests were summarized into a single total score. For comparison with the results of cognitive tests, we applied the Functional independence measure (FIM) at the beginning and at the end of the rehabilitation. Results: The impairments were found mainly on memory subtest (55% of patients). The total score of the short battery of cognitive tests showed high correlation with FIM at baseline (p = 0.68) , as well as with a FIM at the end of the rehabilitation (r = 0.74). The correlations were significantly higher with cognitive FIM than the motor FIM. Conclusion: The brief assessment of cognitive functions after a traumatic brain injury proved to be a diagnostic and predictive valid. Such assessment provides insight into the profile of the impaired functions and enables the follow-up of patients.
