Sažetak
It is well known that a high lipid levels in the blood is acknowledged as a risk factor for cardiovascular diseases that have the highest mortality rate in the world (30 %). The same is in the Croatia where cardiovascular (CV) diseases are leading cause of mortality with share of 49 % in the 2011. Over 60 % of these diseases relates to chronic ischemic heart disease, stroke and myocardial infarction. Although the number of patients with these diagnoses is growing, fees that are paid to hospitals for treatment are reduced due to the Croatian Health Insurance Fund (CHIF) policy. At the same time consumption of statins is increasing and in 2014 compared to 2009 total consumption increased by 40 %. Because of higher consumption of generic drugs pharmaceutical expenses for CV were reduced by 40 per cent. The aims of the research are to examine the correlation of using the statins in reducing CV diseases and number of patient and the morbidity in the period from 2009 to 2014. Further, by interview method to examine whether patients who use statins are performing other lipid reducing measures. Research is conducted in the County of Primorje and Gorski Kotar. Therefore, the main objective is to propose a method of monitoring the effectiveness of prevention programs in order to rationalize the costs of the health system. In the observed period, in spite of increases consumption of statins, the number of patient and morbidity of cardiovascular diseases increases as well. With the survey it was found that statins are used in 79 % of population aged 50-80 with an average of 5 to 20 years. Over 60 % of the population does not change the diet, and also most of the patients are not performing any physical activity or go to the annual medical check-ups. Supplements such as Omega 3 or red rise have been used in 25 % of patients. This confirms the need for better prevention in order to minimize health care expenses but also a better economic evaluation in health policy. Teaching Institute of the Public Health of the County of Primorje and Gorski Kotar carries out training programs in order to promote healthy food and physical activity. Economic monitoring of such programs should not be focused only on costs but on measuring the effectiveness i.e. the outcome and impact of the savings in health care. The proposed input-output-outcome-impact method will be useful tool for measuring efficiency and effectiveness in the short and long term.
