Our Three Dimensional Computer Assisted Functional Endoscopic Sinus Surgery (3D C-FESS)

Aim. The main objective of our team was to design or modify a computer program for 3D imaging of anatomical regions of the head to be surgically treated. Such an approach should allow a considerably better insight into the operating field, and a significantly greater safety of the procedure. Methods. Transmission of video or digital signal from CT device to the computer, transmission of CT-scans using computer network to different locations, special software for 3D modeling-partially created and modified by ourselves for 3D C-FESS, connection of the 3D-digitizer with computer and multimedia navigation through computer during the surgery. Results. Performing our 3D-approach in C-FESS, we were able to "look inside" the patient during surgical planning as well as at the time of the surgery. Conclusion. Perioperative use of the computer-generated 3D operating field model has not found wide application, whereas in FESS it has not yet been used at all. The interactive perioperative use of computer will require appropriate circuit and software support to connect medical instrumentarium to the computer, and to operate the computer via the instrumentarium and advanced multimedial interfaces. Operability of the anatomical regions of the head is determined by computer, thus making FESS really computer-assisted three-dimensional operations (3D C-FESS). By connecting surgical instrumentarium to computer (i.e. "robotic arm"), and operating the computer and multimedial systems during the surgery, this new surgical approach has been completely improved.
