Platelet 5-HT and depressive symptoms in chronic renal patients

The occurrence of depression is a frequent comorbidity in chronic renal patients. The alterations in the central serotonergic system are thought to be related to depression. Platelet serotonin (5-HT) is often used as a peripheral marker for central serotonergic synaptosomes. In the present study the occurrence of depressive symptoms, platelet 5-HT concentration, and correlation between depressive symptoms and platelet 5-HT, were determined in 114 renal patients undergoing hemodialysis (HD), in 65 patients with kidney transplantation (KT), and in 107 healthy gender-matched controls. All patients were divided into groups without, with mild, moderate and severe symptoms of depression, according to the Beck Depression Inventory (1). Depressive symptoms were experienced more frequently in patiens on HD (2/3), than in patients with KT (1/3). There was no correlation between platelet 5-HT concentration and severity of depressive symptoms. Patients on HD had lower platelet 5-HT concentration than patients with KT, and both patients groups had lower platelet 5-HT concentration than healthy controls. The results show reduced platelet 5-HT concentration in patients undergoing HD and with KT, that might reflect impaired peripheral 5-HT function. The greater occurrence of depression in patients on HD than with KT indicate that kidney transplantation improves, besides clinical, also the mental state of chronic renal patients.
