Fibromuscular tissue complex in the pyeloureteral segment as a cause of idiopathic hydronephrosis

On the basis of a histologic and electron microscopic study of the P-U segment of 11 cases of "idiopathis hydronephrosis" the authors found a number of constant and peculiar changes. The most significant changes were the thickening of the connective submucosal layer, the narrowing of the muscle layer with the "longitudinalization" of the fibers, the atrophy of the muscle fibers with clear ultrastructural changes and pinocytosis, a marked proliferation of interstital connective tissue with a reduced number of nexus and membranous changes, and the reduction of the P fibers, with a normal composition and structure of the neural elements. The authors named these changes "fibromuscular complex" and conclude that this complex is responsible for the functional distrubance in the flow of urine through the pyeloureteral segment and, consequntly, for the origin of "idiopathic" hydronephrosis. This new evidencecontributes to the definitive acceptance of the so-called "myogen" theory of the origin of "idiopathic" hydronephrosis and the rejection of the theory which proposed that neurol distrubances play an important role in it. The authors presume that the cause of the development of this "fibromuscular complex" lies in the dysplasia of the connective tissue.
