Sensitivity (Sn), specificyty (Sp), predictive value of a positive (PPV) and negative (NPV) results of flow-volume curve parameters in asthmatic children
Within group of 217 children with mild and moderate asthma (A) aged 10.7±2.2 y. (136 males and 81 females) and control group (C) of 81 healthy children aged 10.4±2.9 y. (40 males and 41 females) we compared parameters of flow-volume curve: FVC, FEV1, FEF75, FEF50, FEF25 and FEV1%FVC. Sn, Sp, PPV and NPV of all parameters were determined. Groups A and C were not different according to the age, body mass and height (p>0.05). The results werw as follows: FVC FEV1 FEF75 FEF50 FEF25 FEV1%FVC Sensitivity (%) 30.6 38.5 60.9 52.2 33.9 9 Specificity (%) 98.4 98.3 77.9 88.7 95 100 PPV (%) 98.2 97.6 88.8 93.1 98.3 100 NPV (%) 32.8 48 41 60.9 14.4 11.2 FVC and FEV1, although according to GINA are main parameters of asthma diagnosis and control in children, showed low Sn and NPV, therefore in our opinion, including FEF75 and FEF50 gave better insight of treatment efficiency. Measuring these parameters we’ll avoid hypodiagnosis and hypotreatment of asthma: from every third for FVC (NPV: 32.8%) or every second for FEV1 (NPV: 48%), on almost everyone for FEF50 (NPV: 60.9%).
