Relation of IgE, ECP and blood eosinophils to bronchial hyperreactivity in asthmatic children

Aim: to investigate relation of total IgE, eosinophil cationic protein (ECP) and peripheral eosinophil counts (PEC) in asthmatic children to bronchial hyperreactivity (BHR) measured with Metacholine challenge test (MCT). Method: 90 children diagnosed with asthma performed MCT using Masterscope Jaeger APS dosimeter for bronchoprovocation, total serum IgE and ECP were measured by Pharmacia CAP system and PEC were counted. Results: 90 asthmatic children (58 m, 32 f), aged 12.4 ±3.1 y, performed MCT according to Guidelines for Metacholine and exercise challenge testing – 1999. Am J Respir Crit Care Med Vol 161. pp309-29. The following modified categories of hyperreactivity were used: PC20(mg/ml) PD20(μg) PD20(μg)cumul No. Interpretation >4 >266.7 >354.2 38 Normal BHR (N-BHR) <1-4 4.2-266.7 4.2-354.2 52 Mild to severe BHR (MS-BHR) According to Kruskal-Wallis - Anova statistical analysis, IgE values in MS-BHR group (MV±SD: 570 ± 622 kU/l) were significantly higher than in N-BHR group (360 ± 558 kU/l) x2: 10.9, p < 0.001. No statistically significant difference was found in ECP between MS-BHR (18.3 ± 12.9 mg/l) and N-BHR group (18.9 ± 20.9): x2: 0.28, p: 0.59, and in PEC between MS-BHR (320 ± 106) and N-BHR group (330 ± 195): x2: 1.42, p: 0.23. Conclusion: Asthmatic children with elevated total IgE (>80 kU/l) have statisticaly significant elevation of bronchial reactivity (MS-BHR ; p<0.001). There is no significant difference in ECP and PEC values between asthmatic children with normal (N-BHR) and elevated (MS-BHR) bronchial reactivity.
