OBESITY RELATED HYPERTENSION: LEPTIN/ADIPONECTIN RATIO AS A POTENTIAL ATHEROSCLEROTIC MARKER
Obesity and excess of visceral fat represents risk factors for hypertension, diabetes and cardiovascular diseases. The common determinator for the above conditions is metabolic syndrome (MetS) together with dislipidemia, where the key component is neuroendocrine activity of fat cells - adipocytokines productions. Leptin and adiponectin are adipose tissue-derived hormones with opposite associations in the MetS and coronary heart disease but without counterregulatory physiologic effect. Leptin plays critical role in regulating food intake, maintaining energy expenditure and regulating immune function. Adiponectin has direct effects on insulin sensitivity, glucose metabolism and central fat distribution. Obese state is characterized by increased leptin and decreased adiponectin level, but its relations with components of MetS are not independent of age and gender. According to research, leptin/adiponectin ratio (LAR) could represents independent marker for early stage of MetS, while there are no clinical manifestations of related diseases and organ damages. Most of the study followed the LAR as a risk factor in diabetic patients with focus on insulin sensitivity and relation to atherosclerotic procesess. The aim of this study is to find out the connection of LAR in hypertensive, overweight patients with myocardial infarction, in order to detect the importance of its valuations in manifest atherosclerotic disease but independent of diabetes and insulin resistance, in early stages of obesity.
