Pathological changes of the femoral head and the joint capsule in coxarthrosis

There are pathological changes of the femoral head and joint capsule in 35 cases with advanced coxarthrosis presented. The patients were treated on the nDepartmenrt of the Orthopedics of the Osijek University Hospital. Total endoprothesis was indicated in all patients. During the surgery, the head and the neck of the femur, together with a part of joint capsule were removed. Overloaded areas are characterized by degenerative alterations such as erosions of the articular cartilage with loss of chondrocytes and appearance of the cystic areas within the subcortical bone. The areas of the decreased pressure are characterized by the osteophyte formation developed from the margins of the articular cartilage. These osteophytes are result of the fibrocartilaginous outgrowths' ossification that was highly vascularized. The joint capsular tissue is characterized by degenerative changes of the collagen fibers and perivascular mononuclear inflammatory infiltrates. The surgically removed femoral head is a highly suitable material for pathohistologic research in coxarthrosis. According to former data and our observations, changes in extent and direction of the load on a joint are supposed to be the ultimate modificator in the mesenchymal cell differentiation. Thus, the destruction of the cartilage is accompanied by osteosclerosis and osteophytosis.
