Magnetic resonance image correlates of cerebral palsy in adolescents with perinatal PVL-PVH lesion
This paper is a part of our long-term study of structural and functional plasticity of the human cerebrum after perinatal lesions. MRI parameters were assessed in 21 adolescents aged 13-16 years with diagnosis of cerebral palsy (defined at the age of 2 years) who suffered periventricular leukomalacia (PVL) or periventricular haemorrhage (PVH) during perinatal period. In all children examined the following MRI correlates of cerebral palsy found: 1) structural rearrangements and changes in signal intensity in the centrum semuovale and related radiations of the cerebral white matter ; 2) localised or general reduction of the corpus callosum ; 3) enlarged lateral ventricles. in 6 children born older than 33 weeks (34-term) additional pathology was found: multilocular cysts or porencephalic cavities in the white matter of cerebrum. This specificity of pathology of cerebrum after 34 weeks we interpret as a lack of structural repair due to the departure of major "waiting" axon systems and possibly differential maturational status of the glia cells. We propose that parasagital lesions of centrum semiovale during developmant cause retrograde-anterograde degeneration of fibres of corpus callosum, reduction of the cerebral white matter and concommitant enlargement of ventricles.
