External rhinoplasty - different technique or different approach
Ever since external rhinoplasty was introduced in the United States and Canada thanks to Professor Padovan large number of rhinoplasty surgeons are using this approach not only in severe or tough cases but as a routine procedure. External rhinoplasty gives more opportunities than internal rhinoplasty to deal with different kind of problems. It is addressed specially to nasal tip. Key component of a successful rhinoplasty has always been control of the nasal tip contour. When we think about reshaping nasal tip through external rhinoplasty, we think more about sculpturing and reorientation of the nasal cartilages in contrast to excising the cartilages. That can be done using different types of sutures (intradomal, transdomal etc.). It is also possible through external approach to put different kind of grafts to make nasal contour appropriate to the face, and at the same time to solve the problem. Transverse columellar scar can be avoided using sharp cuts, appropriate alignment of the skin edges and use of 6.0 nylon suture. If we think about the nose as an envelope (skin subcutaneous tissue muscles) that is covering infrastructure (bones and cartilages) we should encounter that incisions may have a different result in the healing process. In our opinion external rhinoplasty is not just another technique but different approach to nasal structure.
