Immunohistochemical study of renal biopsies in children with idiopathic nephrotic syndrome

Many studies have confirmed the implication of cell-mediated immunity in the pathogenesis of idiopathic nephrotic syndrome(INS).In order to identify the components of cell- mediated immune reaction in situ, we investigated the presence of B, T, T4 and T8 lymphocytes, macrophages and NK cells in renal biopsies from 26 children with INS.The cells were identified by monoclonal antibodies(CD20, CD3, CD4, CD8, CD68 and CD56)and immunoperoxidase labelling. The number of positive cells was expressed per glomerulus and per 1mm2 of interstitial tissue. The four normal sections obtained from kidneys removed for localized tumors, served as controls.There were no positive cells in normal glomeruli, while in glomeruli of children with INS, T lymphocytes, T4 lymphocytes, macrophages and NK cells were found(0.22, 0.23, 0.57 and 2.18 cells per glomerulus respectively). In normal interstitium T lymphocytes, T4 lymphocytes and macrophages were found only(7.7, 0.75 and 5.5 cells per 1mm2 of interstitial tissue respectively), while in interstitium of children with INS all examined cells were found(B lymphocytes 0.60, T4 lymphocytes 2.0, T8 lymphocytes 0.64, macrophages 6.53, NK cells 3.78 cells per mm2 of interstitial tissue. Results of the study show that in kidneys of children with INS the principal cells of delayed hypersensitivity reaction are present.The finding of NK cells both in glomeruli and interstitium is of special interest since these cells have never been previously suspected to participate in immune reactions in INS.
