Bone mineral density in Croatian children with inflammatory bowel disease
Inflammatory bowel disease (IBD) is associated with decreased bone mineral density (BMD) and increased risk for osteoporosis mostly due to undernutrition, prolonged steroid usage and decreased activity. It seems that low BMD occurs at diagnosis in Crohns disease with unclear pathogenesis. So far, it is the first assessment of bone mineral status in croatian children with IBD. The aim of this study was to compare bone mineral density in newly diagnosed IBD patients with that in children with previously diagnosed disease. Lumbar spine (L1–L4) bone mineral density was assessed by dual energy x-ray absorptiometry in 20 patients. 11 were newly recognised, 9 were previously diagnosed. Seven of previously ill patients had systemic steroid therapy. In 20 IBD patients the mean age was 12.6 years (range 9–17). The mean duration of the disease in previously diagnosed patients was 2.15 years (range 1–5 years). In newly diagnosed patients the mean Z score (−0.55, range −3.02 to +1.42) was significantly higher than in previously ill patients (−2.3, range −4.75 to − 1.17). There was no significant difference in Z scores in new Crohns disease patients (−0.62) comparing with new ulcerative colitis patients (−0.38). Significantly lower bone mineral density occurs in children with prolonged inflammatory bowel disease, especially in those with steroid therapy, comparing to BMD in new patients. Our results show that there is no difference in bone mineral density in newly recognised IBD patients comparing to healthy age and sex matched population.
