Intestinal failure-associated liver disease (IFALD) is the most prevalent complication affecting children with intestinal failure receiving long-term parenteral nutrition (PN). We review here the definition, diagnostic criteria, pathogenesis and risk factors. We discuss the role of enteral nutrition, PN and its components, especially lipid emulsions. We discuss the surgical treatment, including intestinal transplantation, its indications, technique and results. We emphasize the importance of specialized intestinal failure centres.
