Radiological and clinical characteristics of partial articular disc displacement in patients with TMJ disorder
Partial displacement of the articular disc implies the existence of displacement in only one of the layers of the sagittal plane within the same joint. The aim of this study was to investigate into the radiological and clinical characteristics of temporomandibular joints in patients with partial disc displacement and to compare them with the joints of patients who suffered from other forms of complete disc displacement (with reduction, without reduction) in the same time period. The retrospective study included consecutively collected data from 92 patients (average age 33.97, predominantely women, 88.43%) who were selected for this study according to professional and technical criteria. After clinical diagnostics, the patients obtained a definitive diagnosis of disc displacement in each joint by magnetic resonance imaging. Metric measurements and quantitative analysis of the displacement were performed according to the Kurita et al. method. Research showed a significant difference in the position of the articular disc between different layers of the same joint with the partial displacement diagnosis (layer with displacement, layer without displacement) (Mann-Whitney U test, p= 0.0000) as well as a significant difference in the condylar position (Mann-Whitney U test, p= 0.0110). There was no difference in position of the articular disc in joints with partial displacement (layer with displacement) with respect to the complete displacement with reduction (Mann-Whitney U test, p=0.6188), however, there was a difference in the position of the condyle (Mann-Whitney U test, p=0.0100). Partial displacement (layer with displacement) and complete displacement without reduction also differ in the position of the articular disc (Mann-Whitney U test, p=0.0209) and the position of the condyle (Mann-Whitney U test, p=0.0168). Qualitative morphological characteristics as well as the analysed clinical parameters have shown that partial displacement is practically the mildest form of articular disc displacement and that it differs significantly from other investigated forms of discopathies.
