Different patterns of altered lipid metabolism in angina pectoris and acute myocardial infarction
Background: Altered lipid metabolism is based on increased intake of fat associated with oxidative modification of lipids, in particular low density lipoproteins (LDL) resulting in oxidative stress (OS). Aiming to evaluate if OS based on lipid peroxidation and production of oxidized LDL (oLDL) is progressively manifested or occurs in the cardiac diseases like angina pectoris (AP) and acute myocardial infarction (AMI). Results: We analyzed parameters of lipid metabolism, amount of lipid peroxidation products (LPO) and antibody titer against oLDL in patients with angina pectoris (AP ; N=32), with AMI (N=36) and in normal healthy subjects (N=52). Although total cholesterol levels and LDL-cholesterol were above normal values only in patients with AP (p<0.001), not in those with AMI (p>0.05), other parameters of lipid metabolism were equally altered in patients with AP and with AMI. Hence, in both groups levels of HDL-cholesterol were significantly lower than in healthy subjects, while triglycerides were increased above normal (p<0.05). However, OS-damaged lipids seem to occur only in patients with AMI. Namely, in these patients significant increase of LPO was detected (p<0.001) while anti-oLDL titer was decreased (p=0.0014). These values were normal in patients with AP (p>0.1). Conclusion: Altered lipid metabolism is in patients with AP manifested at least as much as in patients with AMI, while oxidative damage of lipids, in particular LDL develops mostly in AMI, not in patients with AP. Immune response to this, manifested by the production of antibodies against oLDL appears to play important role in AMI.
