The use of piezoelectric effect in laparoscopic surgery

The recognition and localization of arteries in order to prevent their intraoperative lesions pose a major problem in laparoscopic surgery. Endoscopic pulse detector is a new device that has been specially developed for the detection and recognition of pulsating blood vessels in laparoscopic surgery. The instrument is inexpensive and simple for use. Pulsating signal is identified via an accelerometer housing and a low-noise operational amplifier, and is eventually converted into an auditory, light or graphic signal, by choice. The work and features of the instrument were tested in laboratory conditions by use of simulated circulation. Clinically, the instrument was tested in 20 elective non complicated laparoscopic cholecystectomies. The results of laboratory testing showed the instrument to be reliable in identifying pulsating blood vessels throughout the frequency range of 36-130/min and circulating systolic pressure range of 40-180 mmHg, while the signal amplitude rose with pressure increase. The results of clinical testing showed the use of endoscopic pulse detector in laparoscopic cholecystectomy to significantly reduce the time needed for Calot’s triangle preparation (t = 3,91; df = 38; p < 0,001), also making the cystic artery positioning more reliable. Accordingly, endoscopic pulse detector should be included in the standard laparoscopic surgery instrumentarium, because it significantly contributes to the prevention of intraoperative vascular lesions by reliable identification of pulsating blood vessels.
