Impact of simple hematological parameters on prognosis in Non-Hodgkin's lymphoma : Multivarate analysis with 32 variables on 217 patients
Non-Hodgkin Lymphomas (NHL) are characterized by considerable variability in course and prognosis. A number of prognostic factors have been identified and new independent candidates for predicting prognosis are increasingly reported. To investigate whether the simple hematologic parameters routinely taken at diagnosis are useful to identify uniforme subsets according to prognosis uni- and multivariate analyses were done in 217 newly diagnoses patients with NHL in Clinical Hospital "Mekrur" between 1986. And 1991. According to Kiel classification 142 /65%) patients had high grade, and 75 (35%) had low grade malignancy NHL. 76 patients (35%) were T-cell, 139 (64%) B-cell NHL., 2 (1%) patients unclassified NHL. Median age was 55 years (range 14-87). Male to female ratio was 115/102. At diagnosis, 90 (41%) patients had anemia (hemoglobin<129g/L), 9 (4%) has trombocytopenia (platelets < 100x10Č9/L), and 49 (23%) patients had trombocytosis (plateletas > 400x10Č9/L). Erythrocyte sedimentation rate (ESR) was higher then 40 mm/1 hour in 98 (45%) patients. In an univariate analysis, the best overall survival was associated with ESR (p=0.000), platelets (p=0.0313), hemoglobin concentration (p=0.000), and leukocyte number (p=0.0386). Thirty-two clinical, histological, immunological, hematological, biochemical parameters were entered in Cox multivariate analysis. The overall global chi-square was 84, 098. Simple parameter contributed with over 80% of predicted variability in Cox proportional hazards model. Therefore, simple, readily accessible hemtological parameters are useful in predicting prognosis of NHL and contain considerable prognostic information. We conclude that these parameters with other simple prognostic factors such as age, histopathologically assessed grade of malignancy should be taken into consideration in more sofisticated) tests should for their routine use prove an extra predictive power over simple and reliable parameters.
