Relationship between metabolic syndrome and meal induced glucagon like peptide-1 response in type 1 diabetic patients

Metabolic syndrome (MS) can be found in about 30- 40 % type 1 diabetic (T1DM) patients. The meal induced glucagon-like peptide-1 (GLP-1) secretion in T1DM patients with MS is yet to be clarified. The aim of this study was to analyse the relationship between total fasting GLP-1 level and meal induced GLP-1 response with MS prevalence in T1DM patients compared to lean normal glucose tolerance (NGT) control subjects. Study included 77 T1DM (61% male) patients, 26 (33.76%) with MS, mean age 45.08 years, mean BMI 25.42 kg/m2, median diabetes duration of 21 years and 10 NGT control subjects adjusted for age, gender and BMI. Circulating GLP-1 level was measured before and 30 min after a meal by ELISA using ALPCO Diagnostics kit. The NGT group presented with the significantly higher total fasting, postprandial and meal induced GLP-1 concentrations compared to T1DM groups. The group of T1DM patients without MS showed higher rise in circulating GLP-1 level compared to group with MS. After age, gender, disease duration and meal caloric value adjustment, GLP-1 response levels were inversely associated with MS prevalence in the binary logistic regression model. Higher meal induced GLP-1 response is associated with lower MS prevalence but whether GLP-1 has a protective role in MS development is yet to be evaluated.
