Hypovitaminosis of vitamin D in patient with chronic upper respiratory diseases.

The nasal mucosa is highly specialized immunologic barrier that has evolved to process a constant load of foreign antigens with minimal collateral inflammation. Deep in the epithelium reside a variety of immune cells including lymphocytes and antigen presenting cells, specifically dendritic cells and macrophages. These are key components of the acquired immune respons. Rhinosinusitis occurs when the lining of the sinuses gets infected or irritated, become swollen, and create extra mucus. The swollen lining may also interfere with drainage of mucus. Chronic rhinosinusitis is different from the more common form of rhinosinusitis (called acute rhinosinusitis), which is a temporary infection of the sinuses that often occurs following colds. Chronic rhinosinusitis is a more persistent problem that requires a specific treatment approach. Vitamin D, as an immunomodulatory steroid hormone, directly affects dendritic cell, monocyte, macrophage, B cell and T cell functions. It has been shown that vitamin D have an immunomodulatory effect with a significant impact on immune function. Specifically, vitamin D regulates the mechanisms which suppress the inflamatory response and direct the differentiation fate of immune cells. Vitamin D has been shown to play an important role in asthma, and the concepts of the unified airway model allows the extrapolation of vitamin D as a critical player in chronic rhinosinusitis. The purpose of this study is to discuss the role of vitamin D in patients with chronic upper respiratory diseases.
