The value of ultrasound in acute ankle injurie- comparison with MR

To asses the value of the US in different grades of acute trauma by comparing ultrasonographic with MR findings. We analyzed 30 patients with acute ankle trauma (17 men and 13 women ) with the average age of 33 yrs (16 – 66) without visible bone fracture on standard radiograms. One week after injury all patients were examined by US. We used an 8-15 MHz linear probe. 10 days later the patients were examined by MRI. The anterior talofibular ligament was found normal using US in 20.6% of the patients and in 20.3% of the patients using MRI. Ligament lesions were found by ultrasound in 40% of the cases and confirmed by MR in only 20.6% of the cases. Ultrasound diagnosed ruptured anterior talofibular ligament in 33.3% of the cases, and MR in 50 % of the cases. In 80.3% of the cases, the calcaneofibular ligament appeared to be intact with both methods. Ultrasound showed ligament stretching in 10.6% of the cases and MR proved that in 10% of the cases. In other 6.6% of the cases, MR revealed complete ruptures. Intraarticular effusion was found in 80.3% patients examined by US and in 86.6% examined by MR. A lesion of the long peroneal muscle tendon was found in 40.6% of the patients with both methods. A lesion of the short peroneal muscle tendon was found in 33.3% of the lesions and proved by MR in only 20.3% of the cases. In other patients findings were normal. US found lesions of the anterior tibial muscle tendon in 10% of the patients and MR in 10.3% of the patients. US found 10.6% of lesions of the long halucis flexor tendon as compared to MR in 20% of the cases. US proved to be a good and reliable method for diagnosing grade I and II ankle sprains, but for proper evaluation of grade III lesions MRI is recommended.
